RN MDS Coordinator

Our ideal candidate must possess an RN degree / license and have a minimum of two years nursing experience in a health care setting, long term care and supervisory experience preferred.
Job duties include but are not limited to, developing, conducting and completing resident assessments timely and accurately determines and creates a schedule for timely completion of resident assessments and leads the coordination with the interdisciplinary team.
oversees the completion of resident assessments timely and accurately within required timeframes.
partners with the DON to create and implement procedures affecting MDS procedures such as the notification of new admissions Garners feedback from other department managers regarding resident assessment functions of the facility, as well as evaluate the efficiency and effectiveness of the MDS procedures.
Monitors, reviews, and approves assessments to ensure accuracy, and timely completion, as well as educate and train staff on the MDS requirements and supporting documentation.
Participates in the discharge planning process Participates in state and federal survey process.
Provides direct nursing care as necessary Participates in resident and family care plan meetings Job Type:
