This position is in Bay County and requires in-office work as well as home visits to support new families from prenatal to age 3 years old.

Qualifications:
This position requires a bachelor's of science in social work or other human service related field, or equivalent experience to be over five years in care coordination and/or case management related to children and maternal health.
Care Coordinator should work independently as well as possess strong writing, communication, organization, time management, and computer skills.
This position values experience working with infant and toddler aged population, knowledge of case management, and child development.
Additional preferred qualifications include, experience working within the system of care providing services to prenatal clients.
A current, valid Florida Driver's license, current automobile insurance, and a reliable car are essential for this position as some travel may be required.
This is a public health social work position with the responsibility for managing a caseload of pregnant women and infants at risk in the Healthy Start Program.
Responsibilities of this position are accomplished through planning, direct service, intervention, evaluation, and education.
Primary Functions Completion of recommended training.
This includes:
certification in breastfeeding counseling, developmental screens, parenting programs, smoking cessation and motivational interviewing.
Conduct home visits for the purpose of providing assessments, developmental screenings of high risk pregnant women and infants, development of family support plans, provide education and support, enhance the client's ability to access care and provide emotional and parenting skills to reduce risks associated with infant mortality and morbidity.
Develops, implements, monitors and modifies care coordination and family support plans for Healthy Start pregnant women and infants through home visits, education and referrals.
Provides prenatal/childbirth education, smoking cessation, basic newborn education and breastfeeding education and support, family planning education, immunization information, psychosocial evaluations, and referrals.
Services are offered through home visiting to homes, clinics, correctional facilities and substance abuse facilities.
Developmental screening services are provided for all, with particular attention to preterm and substance exposed babies.
Works collaboratively with multi-disciplinary teams when needed.
Assess clients and family needs, provide referrals and utilize family support plans when appropriate.
Completes documentation as required in Well Family System and client records to reflect the type and amount of service provided.
Communicates with health care providers regarding the client's needs, progress and referrals given.
Secondary Functions:
Participates in staff, in-service/continuing education meetings to increase skills and knowledge.
Participates in quality improvement activities.
Serves as a resource person to community and health care agencies.
Participates in community projects, meetings, and health fairs to promote public awareness and education.
Performs other related duties as required.

The check will bounce and the victim is left responsible.
